Home Health Case Manager RN

As a Home Health Case Manager Registered Nurse with CentraCare, you will have the opportunity to provide skilled care to patients in their homes. This role requires strong assessment and communication skills, as well as the ability to work independently and as part of a team.
Responsibilities- Provide skilled care to patients in their homes, including wound management, infusion therapy, and education and training on new diagnoses.
- Conduct assessments and develop care plans to meet the unique needs of each patient.
- Collaborate with other healthcare professionals to ensure seamless care coordination.
- Communicate effectively with patients, families, and caregivers to ensure understanding and compliance with care plans.
- Document patient information and progress in a timely and accurate manner.
- Graduate from an accredited school of nursing required.
- Current Minnesota Registered Nurse license required.
- Current American Heart Association (AHA) BLS Healthcare Provider card required.
- Minimum of one year of RN experience in long-term care or an acute care setting preferred.
- Reliable transportation required.
- Valid Minnesota driver's license, a driving record in good standing, and insurance liability coverage required.
CentraCare offers a competitive salary, milage reimbursement, and extra pay for working weekend shifts.
