Care Transitions Director

Overview
Physician Affiliate Group of New York (PAGNY) is seeking a highly skilled Director of Care Transitions to join their team at NYC Health + Hospitals/South Brooklyn Health. This individual will be responsible for managing care transition plans for inpatients referred by community providers, ensuring effective and transparent communication with clinical and operational leadership, and providing clinical administrative guidance to medical staff.
Key Responsibilities:
- Work with the Department of Medicine Chairperson and Director of Community Medicine to provide clinical direction to direct reports and community providers.
- Develop and collect performance metrics to plan, implement, or administer quality patient care programs.
- Review, approve/deny, and sign off on leave of absence requests, timesheets, and evaluations for mid-level providers.
- Provide direct care to patients assigned to the unit.
Qualifications:
- A current, valid, and unrestricted license to practice medicine in accordance with the laws of the State of New York.
- Board Certification in Internal Medicine.
- Minimum 5 years' experience practicing in the medical specialty.
- Minimum 3 years' supervisory experience, including demonstrated leadership and experience with personnel, finance, quality, and clinical resource management.
