Key Responsibilities:
  • Engage in transition planning and needs assessment to ensure a smooth transition for participants.
  • Monitor discharge and ongoing care planning to ensure participants receive the necessary support.
  • Build collaborative relationships with community providers to ensure seamless care coordination.
  • Provide short-term counseling and risk assessment to identify and address potential issues.
  • Resolve clinical issues affecting housing stability and provide crisis intervention and support services as needed.
  • Assist with appointment navigation and referrals to ensure participants receive the necessary care.
  • Conduct comprehensive psychosocial assessments to identify participants' needs and develop individualized plans.
  • Monitor participant progress and goal achievement to ensure successful transition to the community.
  • Conduct family conferences and psychoeducation to support participants and their families.
Qualifications:
  • Masters degree in Social Work or Mental Health Counseling or a related field required (LMSW, LCSW, LMHC).
  • Knowledge of mental illness, substance use disorders, and NYC behavioral health programs.
  • Knowledge of treatment and rehabilitation programs.
  • Ability to develop, evaluate, implement, and modify a clinical treatment intervention to meet the needs of individual recipients.

This is a full-time position located in Brooklyn with a hybrid schedule offering inpatient, home, and community visits. The ideal candidate is a self-starter who can work independently as well as within a team and has excellent follow-up skills. A competitive benefits package is included, with a salary of $70,000.
