Job Title: Clinical Social Worker - Hospice MSW

At Tufts Medicine Care at Home, we are seeking a skilled Clinical Social Worker to join our team. As a Clinical Social Worker, you will provide a range of clinical social work services to patients and families in inpatient, ambulatory, and community settings.

About the Role:
  • Provide psychosocial assessments and interventions in crisis situations, including issues of suicide, homicide, family violence, elder or child abuse.
  • Collaborate with the healthcare team to promote a smooth, coordinated plan of care.
  • Provide medical social services to patients' family members or caregivers on a short-term basis when necessary.
  • Treat patients and families with dignity and respect at all times.
Requirements:
  • Master's degree in Social Work from an accredited institution.
  • Licensed Clinical Social Worker (LCSW).
  • One (1) year of related experience.
