Job Summary

VOLUNTEERS OF AMERICA SOUTHEAST LOUISIANA is an EEO employer - M/F/Vets/Disabled. We are seeking a highly skilled and compassionate Care Coordinator III to oversee our Supportive Services' scattered site Permanent Supportive Housing program in the community.

Key Responsibilities
  • Develop, implement, and monitor person-centered housing support plans to ensure consumers receive services as described in contract.
  • Ensure compliance with weekly performance standards of billing 100 units/25 hours per week for 52 weeks.
  • Accurately recordkeeping, including intake, service planning, service delivery, and case closure.
  • Prepare and complete timely reports and documentation for billing of services as required by Volunteers of America Southeast Louisiana and regulatory agencies/funding sources.
  • Perform home visits, ensure eligibility of services prior to performing services, and off-site case management meetings.
  • Assist consumers/members in accessing and maintaining entitlements/benefits and meeting obligations of tenancy.
  • Maintain assigned up-to-date client files and program records.
  • Participate in training required by accreditation standards, performance, and quality improvement efforts.
Requirements
  • Master's degree in social service field (qualifying degrees include: counseling, social work, psychology, sociology, rehabilitation services, special education, early childhood education, secondary education, family and consumer sciences, criminal justice, or human growth and development).
  • At least one year post-experience working with individuals who are of the eligible population in a community setting.
  • Excellent verbal and written communication skills and ability to function as part of the team.
  • Possess polished and diplomatic customer relation skills verbally, in person, through telephone communications, and in writing.
Physical Requirements

This employee's work is performed at various locations where services are provided to consumers. Incumbent must be able to handle the demands of extensive driving throughout the metro service area.
