The Care Navigator plays a pivotal role in shaping the patient experience, serving as the initial point of contact for patients and guests. This position requires an exceptional communicator who proactively addresses any concerns and ensures that all pertinent information is relayed to the care team. The Care Navigator is committed to delivering exceptional service that delights and surprises our patients.

This role is designed as a per diem opportunity to fill in during clinic scheduling gaps. The operational hours are Monday through Friday from 8 AM to 8 PM and Saturdays from 8 AM to 5 PM. We seek individuals who can accommodate shifts that include 7:30 AM to 4 PM and/or 11:30 AM to 8 PM.

ESSENTIAL JOB FUNCTIONS

- Responds to all patient inquiries with professionalism, accuracy, and timeliness.

- Acts as the first point of contact for patients entering the wellness centers, ensuring a smooth and positive check-in experience. This includes collecting and verifying insurance information and co-pays.

- Manages all guest interactions with the utmost hospitality and professionalism, addressing special requests whenever feasible, resolving complaints, and assisting with inquiries related to clinic services and operational hours.

- Operates a multi-line phone system, professionally handling calls, scheduling appointments, and maintaining effective communication with patients and staff.

- Maintains patient records within electronic health record systems by updating information and uploading relevant documents.

- Processes incoming and outgoing packages, faxes, messages, and mail accurately and promptly.

- Completes all necessary transactions to finalize the patient visit.

- Assists with additional administrative responsibilities as needed.

- Upholds the highest standards of confidentiality.

- May involve program coordination duties as assigned.

QUALIFICATIONS AND REQUIREMENTS

- Minimum of two years of experience in a related role (client-facing, hospitality, or sales).

- Exceptional communication and interpersonal abilities.

- Strong organizational skills.

- Capacity to function effectively as a key team member.

- High level of professionalism.

- Proficient computer skills and familiarity with Apple products (iMac, iPad, iPhone).

- Proficient reading, writing, and oral communication skills in English.

- Ability to swiftly learn new procedures and processes.

- High School diploma required; an Associate's Degree is preferred.
