The Care Navigator plays a pivotal role in shaping the patient experience, whether it is the initial phone call or the first encounter with a team member at the center. This position requires an exceptional communicator who proactively addresses any concerns and shares pertinent information with the care team. The Care Navigator is dedicated to providing exceptional service that delights and surprises patients.

This role is designed as a per diem opportunity to fill in during clinic schedule gaps. The operational hours are Monday through Friday from 8 AM to 8 PM and Saturdays from 8 AM to 5 PM. We seek individuals who can work shifts that include 7:30 AM to 4 PM and/or 11:30 AM to 8 PM.

ESSENTIAL JOB FUNCTIONS

1. Responds to all patient inquiries with professionalism, warmth, and accuracy.

2. Serves as the first point of contact for patients entering the wellness centers, ensuring a smooth and positive check-in experience. This includes collecting and verifying insurance information and co-pays.

3. Manages all patient interactions with the utmost hospitality and professionalism, accommodating special requests when feasible, resolving complaints, and assisting with inquiries related to clinic services and hours.

4. Operates a multi-line phone system, answering calls professionally, scheduling appointments, and ensuring effective communication with patients and staff.

5. Maintains patient records within EHR systems by updating information and uploading relevant documents.

6. Processes incoming and outgoing packages, faxes, messages, and mail accurately and promptly.

7. Completes all necessary transactions to finalize the patient visit.

8. Assists with additional administrative tasks as assigned.

9. Upholds the highest standards of confidentiality.

10. Duties may include program coordination as assigned.

QUALIFICATIONS AND REQUIREMENTS

1. Minimum of two years of experience in a related role (client-facing, hospitality, or sales).

2. Exceptional communication and interpersonal skills.

3. Strong organizational abilities.

4. Capacity to function effectively as a key team member.

5. High level of professionalism.

6. Proficient computer skills and familiarity with Apple products (iMac, iPad, iPhone).

7. Proficient reading, writing, and oral communication skills in English.

8. Ability to quickly grasp new procedures and processes.

9. High School diploma required; AA Degree preferred.
