POSITION SUMMARY: The Cupertino Healthcare & Wellness Center is seeking a dedicated professional to deliver direct patient care and support initiatives that enhance the performance of the therapy department.

KEY RESPONSIBILITIES:

The role involves aiding in the recovery and improvement of independent functioning for individuals affected by health issues or injuries. Responsibilities include:

  • Conducting thorough patient assessments.
  • Creating and implementing effective treatment plans with necessary approvals.
  • Overseeing the work of Physical Therapy Assistants and aides in patient care.
  • Maintaining communication with supervisors and interdisciplinary team members regarding patient status and treatment strategies.
  • Educating patients' families and nursing staff on follow-up care.
  • Gathering information from patients and families about their prior functioning, lifestyle, and recovery expectations.
  • Utilizing resources for effective problem-solving and best clinical practices.

DOCUMENTATION:

Ensure accurate and comprehensive documentation in line with regulatory and accreditation standards, including:

  • Recording evaluations, treatment notes, and discharge summaries.
  • Documenting treatment charges accurately to align with billing data.
  • Writing clear, objective documentation that outlines patient needs and progress.
  • Managing the Medicare Medical Review and Denials process in collaboration with relevant staff.
  • Obtaining necessary consents for treatment.

INTERDISCIPLINARY COLLABORATION:

Demonstrate a commitment to a collaborative approach to patient care by:

  • Incorporating insights from the interdisciplinary team into treatment assessments.
  • Engaging with patients and families to inform treatment recommendations.
  • Contributing to team meetings and conferences.
  • Educating team members in areas of expertise.
  • Facilitating effective communication with discharge planners.

QUALIFICATIONS:

Skills and Knowledge:

  • Expertise in Physical Therapy evaluations and treatments.
  • Ability to supervise and guide assistants and aides.
  • Understanding of physical modalities used in therapy.
  • Familiarity with prosthetic and orthotic devices.
  • Knowledge of medical terminology.
  • Strong communication skills.

EDUCATION:

Bachelor's, Master's, or Doctorate Degree in Physical Therapy from an accredited institution.

EXPERIENCE:

Preferred experience in geriatric rehabilitation and adult neurological or cardiac rehabilitation.

PHYSICAL REQUIREMENTS:

Must be capable of lifting and transferring patients frequently throughout the day and performing physical tasks related to patient care.

CERTIFICATION:

Current Physical Therapist Registration/State Licensure and CPR certification are required.
