Position Summary:

This role involves delivering professional nursing care to patients in a primary care setting, adhering to nursing practices, agency policies, and regulations.

Key Responsibilities:

  • Plan, deliver, supervise, and document nursing care utilizing the nursing process in accordance with the NC nurse practice act and physician orders.
  • Employ professional nursing judgment to tailor care plans based on patient assessments.
  • Conduct comprehensive assessments to prioritize care and set achievable outcomes.
  • Educate patients and families about health alterations and care plans.
  • Develop and implement effective discharge plans from the onset of services.
  • Gather or create educational resources for patients and families, implementing and evaluating educational interventions.
  • Exhibit professional nursing conduct, communication, and ethical decision-making.
  • Participate in interdisciplinary team meetings and case conferences to ensure quality care.
  • Maintain up-to-date knowledge of Medicare/Medicaid/JCAHO standards through ongoing education.
  • Document patient care accurately and in a timely manner.
  • Oversee medication reconciliation and educate patients to prevent adverse events.
  • Communicate regularly with team members, caregivers, and physicians.
  • Adhere to infection control standards and utilize personal protective equipment as required.
  • Delegate and supervise care provided by nursing assistants and licensed practical nurses.

Professional Development: Identify personal learning needs and participate in staff development programs to enhance effectiveness.

Work Environment: This position primarily involves patient care in residences or skilled nursing facilities, requiring travel to and from patient locations.

Qualifications:

  • Associate Degree or Diploma in Nursing required.
  • Current RN license in NC or compact RN licensure from a participating state.
  • CPR certification from the American Heart Association.
  • 1-2 years of experience in med-surgical, critical care, or home health/hospice preferred.
  • Valid driver's license and current automobile insurance.

Skills and Abilities: Knowledge of Medicare and Medicaid regulations, nursing processes, and the ability to assess patient needs effectively. Strong communication skills and the ability to work independently in various environments are essential.
