About the Role

We are seeking a skilled RN-Care Coordinator to join our team at Access Dubuque. As a key member of our healthcare team, you will play a vital role in coordinating patient care and ensuring seamless transitions between healthcare settings.

Key Responsibilities
  • Care Coordination: Screen adult medical/surgical in-patient and observation patients, assess their health status, and identify needs for referrals to multi-disciplinary services.
  • Patient Advocacy: Prioritize patients for care coordination, monitor and coordinate an interdisciplinary plan of care, and communicate changes to patients and families.
  • Education and Support: Provide education to patients and families on available options for care through the continuum, and facilitate empowerment of patients and families in self-management and healthcare decision-making.
  • Utilization Management: Apply utilization management plans, communicate with healthcare teams and patients on financial aspects of care, and identify and manage over- and under-utilization of services.
Requirements
  • Education: Graduate of an accredited nursing program (BSN within 5 years of hire)
  • Experience: Two years of clinical experience in focused areas working with multidisciplinary teams
  • License/Certification: Current licensure in good standing to practice as a Registered Nurse in Iowa, Certification in Case Management
