The Clinical Care Navigator plays a crucial role in integrating and coordinating the healthcare journey of individuals.

Key Responsibilities
Care Coordination
+ Conduct comprehensive assessments of adult patients to evaluate their health status, support systems, and resource needs.
+ Prioritize patients for care coordination based on established criteria.
+ Monitor and coordinate an interdisciplinary care plan in collaboration with patients and their support systems across the healthcare continuum.
+ Collaborate with the Social Work team to develop effective discharge plans for complex cases.

+ Work with both internal and external resources to ensure safe and timely transitions of care.
+ Participate in daily interdisciplinary rounds to plan and evaluate patient-focused care.
+ Document care management plans, including clinical needs and resource utilization, in a clear and timely manner.
+ Integrate with ambulatory care management teams, particularly for high-risk patients.
+ Educate patients and families about available care options throughout their healthcare journey.

Plan of Care/Patient Advocacy
+ Oversee the assessment, development, and implementation of individualized care plans.
+ Assess patient responses to treatment and ensure that care plans are accessible and understood by patients and families.
+ Communicate any changes in care plans promptly and negotiate healthcare resources on behalf of patients.

Education
+ Provide education to patients and families to support the development of care plans.
+ Collaborate with direct care nurses to address patient and family learning needs.

Qualifications
Minimum Requirements
+ Graduate of an accredited nursing program (BSN required within 5 years of hire).
+ Two years of clinical experience working with multidisciplinary teams.
+ Current licensure as a Registered Nurse in good standing.

+ Strong communication skills in English.
+ Multicultural sensitivity and basic computer proficiency.
