About the Role

We are seeking a highly skilled MDS Coordinator to join our team at Manor Care of Dubuque. As a MDS Coordinator, you will play a critical role in ensuring our residents receive the highest standard of care by developing, monitoring, auditing, and modifying each resident's care plan to meet their unique needs and goals.

Key Responsibilities
  • Develop and implement resident care plans that meet individual needs and goals
  • Perform resident assessments and assist in the discharge process
  • Collaborate with healthcare teams to ensure seamless care delivery
  • Monitor and audit care plans to ensure compliance with regulatory requirements
  • Provide education and training to staff on care planning and delivery
Requirements
  • Unencumbered State of Iowa R.N. License
  • Graduate of an accredited nursing program
  • C.P.R. Certification
  • Prior experience as a MDS Coordinator, R.N. in a Skilled Nursing Facility setting, or C.N.A. is preferred
