Key Responsibilities
Care Coordination
- Conduct comprehensive assessments of adult patients to evaluate their health status, support systems, and resource needs.
- Prioritize patients for care coordination based on established criteria.
- Oversee and manage an interdisciplinary care plan in collaboration with patients and their support systems.
- Work closely with social work teams to develop effective discharge plans for patients with complex needs.
- Participate in daily interdisciplinary rounds to ensure patient-centered care delivery.
- Document care management plans, including clinical needs and resource utilization.
- Collaborate with ambulatory care teams, particularly for high-risk patients.
- Educate patients and families about available care options throughout the healthcare continuum.
- Advocate for patients and payers to achieve optimal outcomes.
Patient Advocacy and Plan of Care
- Develop and implement individualized care plans based on thorough assessments.
- Communicate effectively with patients and families regarding care goals and progress.
- Identify and negotiate necessary resources for optimal patient care.
Qualifications
Minimum Requirements:
- Graduate of an accredited nursing program (BSN required within 5 years of hire).
- Two years of clinical experience in relevant areas.
- Current licensure as a Registered Nurse in good standing.
Skills and Abilities:
- Strong communication skills, both verbal and written.
- Ability to work collaboratively in a multicultural environment.
- Proficient in basic computer applications.
- Critical thinking and independent decision-making skills.
