Key Responsibilities

Care Coordination

  • Conduct comprehensive assessments of adult patients to evaluate their health status, support systems, and resource needs.
  • Prioritize patients for care coordination based on established criteria.
  • Oversee and manage an interdisciplinary care plan in collaboration with patients and their support systems.
  • Work closely with social work teams to develop effective discharge plans for patients with complex needs.
  • Participate in daily interdisciplinary rounds to ensure patient-centered care delivery.
  • Document care management plans, including clinical needs and resource utilization.
  • Collaborate with ambulatory care teams, particularly for high-risk patients.
  • Educate patients and families about available care options throughout the healthcare continuum.
  • Advocate for patients and payers to achieve optimal outcomes.

Patient Advocacy and Plan of Care

  • Develop and implement individualized care plans based on thorough assessments.
  • Communicate effectively with patients and families regarding care goals and progress.
  • Identify and negotiate necessary resources for optimal patient care.

Qualifications

Minimum Requirements:

  • Graduate of an accredited nursing program (BSN required within 5 years of hire).
  • Two years of clinical experience in relevant areas.
  • Current licensure as a Registered Nurse in good standing.

Skills and Abilities:

  • Strong communication skills, both verbal and written.
  • Ability to work collaboratively in a multicultural environment.
  • Proficient in basic computer applications.
  • Critical thinking and independent decision-making skills.
