Overview:

The Corporate Care Liaison plays a vital role in managing relationships with designated healthcare facilities and referral sources under the guidance of the Chief Executive Officer and the Corporate Care Liaison Team Leader.

Key Responsibilities:

  • Conduct thorough patient evaluations and facilitate seamless transitions of care into ABCM Corporation's facilities.
  • Perform pre-admission and readmission assessments, ensuring all necessary medical documentation is collected to comply with regulatory standards for continuity of care.
  • Engage with prospective patients and their families to provide comprehensive information about care center offerings, clarify services, assist with financial inquiries, and address any questions.
  • Maintain ongoing communication with Iowa acute care hospitals and ABCM care centers regarding anticipated discharges to skilled nursing and long-term care facilities.
  • Collaborate with nursing staff at care centers to identify potential resident needs and ensure that all required supplies and equipment are prepared for incoming admissions.
  • Develop and nurture relationships with referral sources, providing education and marketing support to enhance collaboration.

Qualifications:

  • Possession of an active and unrestricted Iowa (or compact state) RN or LPN license, or a Bachelor’s degree in Social Work or Human Services (relevant work experience may substitute for educational requirements).
  • Prior experience in long-term care or acute care settings, along with familiarity with Medicare and Medicaid regulations, is preferred.
  • Demonstrated compassion, attention to detail, and exceptional communication and interpersonal skills are essential.
  • Flexibility to work varied hours, including weekends and on-call shifts, is required.
  • Reliable transportation for travel between facilities is necessary.

Additional Information:

Job Type: Full-Time

Salary Range: $60,000 to $70,000 annually, negotiable based on experience.
