Job Summary

Brightways Counseling Group is seeking a highly skilled Care Coordination Specialist to join our team. As a key member of our care coordination team, you will play a critical role in ensuring seamless transitions of care for our clients. This is an exciting opportunity to work with a dynamic team and make a meaningful impact in the lives of our clients.

Key Responsibilities
  • Coordinate care between Brightways and community partners, including Primary Care and other medical professionals.
  • Maintain all established and establish new resources for rapid completion of step-ups in designated territories.
  • Provide 'right now response' to community partners, with all calls and texts returned immediately.
  • Monitor new client consultations and communicate with community partners regarding referred clients that Brightways is unable to accept.
  • Ensure pre-determined metrics are consistently hit, including turn-around-time on step-ups, referrals by service type and location, and other tracked data.
Requirements
  • Bachelor's degree required.
  • Minimum of three (3) years' experience with coordination of care.
  • Minimum of three (3) years' experience with customer service or sales.
  • Experience utilizing a collaborative approach with peers and direct supports.
What We Offer
  • Competitive salary range: $65,000 - $75,000 DOE.
  • (4) weeks of Paid Time Off (PTO) per year, pro-rated amount based on hire date.
