RN Care Coordinator

The RN Care Coordinator is a vital member of the healthcare team, responsible for ensuring the seamless transition of care for patients from hospital discharge to follow-up appointments. This role requires strong communication and organizational skills, as well as the ability to work collaboratively with hospital and other PPG staff.
Key Responsibilities:- Gather and review patient information, including medical, social, and family history.
- Review Health Risk Assessments to identify potential barriers, risk factors, and opportunities for education and preventive services.
- Triage phone calls and coordinate care with providers and other service lines/specialties.
- Provide personalized health advice and referrals as appropriate for recommended testing or preventive counseling.
- Update Electronic Health Records to reflect current patient information.
- Establish and maintain positive working relationships with key customers, including physicians, office staff, patients, families, and interdisciplinary teams.
- Engage patients in self-management and provide timely, clinical documentation of patient encounters.
- Monitor patient charts for test results and communicate results to patients in a timely manner.
- Coordinate patient procedures and possible hospital admissions.
- Participate in patient clinical appointments and assist in patient care.
- Review and queue patient medications for provider review and signature.
- Complete special projects as assigned by leadership team.
- Graduation from an accredited nursing program.
- Active Registered Nurse License in the state of assigned practice.
- ACLS and BLS certification.
- 1 year of relatable or equivalent experience.
- Bachelor's of Science in Nursing.
- EPIC experience.
- 2+ years of relatable experience.
- Frequent sitting and computer use.
