Brightways Counseling Group is seeking a highly skilled Care Coordination Specialist to join our team. As a key member of our care coordination team, you will play a critical role in ensuring seamless care transitions between our organization and community partners.
Key Responsibilities- Coordinate Care Transitions: Work closely with our clinicians and community partners to ensure timely and effective care transitions, including referrals and step-ups.
- Develop and Maintain Partnerships: Establish and maintain strong relationships with primary care providers and other medical specialists to access services, engage clients, and facilitate closed-loop referrals.
- Provide Exceptional Customer Service: Respond promptly to community partner inquiries and provide timely updates on client consultations and referrals.
- Monitor and Analyze Data: Track and analyze data on referrals, turn-around-time, and other key metrics to inform strategic planning and resource allocation.
- Identify New Opportunities: Proactively identify new potential referral sources and community partners to expand our network and improve care coordination.
- Education: Bachelor's degree required.
- Experience: Minimum of three years' experience in care coordination, customer service, or a related field.
- Skills: Excellent communication, organizational, and analytical skills; proficiency in Microsoft Office Suite and related software.
- Competitive Compensation: $65,000 - $75,000 DOE.
