Job Title: Care Coordinator

Marion County, OR is seeking a highly motivated and compassionate Care Coordinator to join our team. As a Care Coordinator, you will play a vital role in providing support and guidance to individuals and families in need of mental health and addiction services.

Job Summary:

The Care Coordinator will work closely with a multidisciplinary team to provide intensive community-based treatment and support services to individuals and families. This includes developing and implementing individualized care plans, providing case management, and facilitating team meetings to ensure the best possible outcomes for our clients.

Responsibilities:

  • Develop and implement individualized care plans in collaboration with clients, families, and healthcare providers
  • Provide case management services to ensure seamless transitions between levels of care
  • Facilitate team meetings to ensure effective communication and collaboration among team members
  • Provide education and support to clients and families on mental health and addiction issues
  • Collaborate with community-based services to ensure comprehensive care
  • Document all interactions and services provided in a timely and accurate manner

Requirements:

  • Three years of relevant work experience in a behavioral health services field
  • Bachelor's degree in a behavioral science field or related field
  • Valid driver's license and acceptable driving history
  • Ability to pass a criminal background check and pre-employment drug screen

Preferred Qualifications:

  • Credentialing as a Qualified Mental Health Associate (QMHA)
  • Experience working with youth populations (birth to 17)
  • Experience working with adult populations (18 and above)
