Job Overview - Clinical Pharmacy Specialist

Position Summary

The Clinical Pharmacy Specialist will act as a vital resource within their designated area, representing the pharmacy department in all facets of patient care. This role encompasses the evaluation of pharmacy and institutional policies related to medication management and ensuring departmental compliance with regulatory standards.

Key Responsibilities

  • Oversee formulary management, including the creation of clinical guidelines and protocols.
  • Provide drug-related education and collaborate with various hospital departments to assess and enhance pharmacy services.
  • Assist in the training and evaluation of clinical staff and learners, including competency assessments.
  • Engage in academic scholarship through authorship, presentations, and participation in national conferences.
  • Support the pharmacy team in collaborative practice initiatives.

Qualifications

Education
Doctor of Pharmacy (Pharm.D.) is required.

Experience
A minimum of 2 years of pharmacy experience in a pediatric, specialized, or clinical setting is required. Candidates without a PGY2 residency must have at least 6 years of experience.

Licenses and Certifications
Eligible for licensure as a pharmacist in the District of Columbia, with potential requirements for Maryland and Virginia. Board Certification in a relevant specialty is also required.

Functional Accountabilities

Order Management and Compliance

  • Interpret and verify medication orders, ensuring accuracy in dosing and administration.
  • Conduct unit inspections to verify compliance with accreditation standards.
  • Collaborate with nursing staff and other healthcare providers regarding medication dispensing systems.

Clinical Interventions and Documentation

  • Monitor drug therapy and intervene as necessary to optimize patient outcomes.
  • Document clinical interventions and maintain accurate patient records.
  • Participate in medication reconciliation to ensure comprehensive medication lists.

Staff Development

  • Assist in the education and supervision of pharmacy students and residents.
  • Conduct training sessions for new pharmacists and provide educational resources.

Quality Improvement Initiatives

  • Lead initiatives aimed at improving clinical pharmacy services and reducing medication errors.
  • Contribute to the development of electronic order sets and clinical pharmacy programs.

Professional Engagement

  • Participate in research projects and contribute to scholarly publications.
  • Engage in professional development activities to enhance clinical expertise.

Organizational Commitment

  • Demonstrate responsiveness to customer needs and maintain a collaborative work environment.
