Job Summary

The Activities Coordinator position at AltaMed is a key role in our Day Center, responsible for overseeing the planning and implementation of activities and social programs for frail elderly and functionally impaired older adults. As a member of the PACE Interdisciplinary team, the Activities Coordinator will participate in care coordination activities, including assessments and care planning.

Key Responsibilities

  • Coordinate and supervise day center activities and social programs, ensuring a planned schedule of social and purposeful independent or group activities.
  • Develop and implement daily, monthly, and special events activities, therapeutic activity programs to improve or maintain physical, cognitive, social, emotional, and spiritual functioning.
  • Utilize various techniques, including arts, crafts, reading, sports, games, dance, movement, drama, music, and community outings, to engage participants.
  • Collaborate with the Activities Team to ensure the activity program meets the needs of participants.
  • Participate in care coordination activities, including assessments and care planning, as a member of the PACE Interdisciplinary team.

Requirements

  • High School diploma required.
  • Associates degree or Bachelor's degree in a health-related field preferred.
  • Minimum of 3 years of experience working with the elderly in a social or recreational program required.
  • Completion of the Activity Leader course designed specifically for this position and approved by the State of California required.
  • BLS certification required.
  • Bilingual: English/Spanish/Mandarin/Cantonese depending on location preferred.
  • Experience in providing virtual activities preferred.
