Job Summary

We are seeking a highly skilled and compassionate Registered Nurse to join our team as a Nurse Care Coordinator. In this role, you will be responsible for providing and coordinating interdisciplinary patient care, utilizing the nursing process to ensure the highest level of care for our patients.

Key Responsibilities
  • Interdisciplinary Patient Care: Collaborate with healthcare teams to develop and implement individualized care plans that meet the unique needs of each patient.
  • Coordinating Care: Facilitate communication between patients, families, and healthcare providers to ensure seamless care transitions and optimal patient outcomes.
  • Assessment and Planning: Conduct thorough assessments of patients' physical and emotional needs, and develop plans to address these needs in collaboration with the healthcare team.
  • Education and Support: Provide education and support to patients and families to promote health and wellness, and empower them to take an active role in their care.
  • Documentation and Communication: Maintain accurate and timely documentation of patient care, and communicate effectively with healthcare teams to ensure that all necessary information is shared.
Requirements
  • Education: Graduate of an accredited nursing program, with a Bachelor of Science in Nursing required within 5 years of hire.
  • Experience: 1 year of relevant experience in a healthcare setting.
  • Licensure: Licensed to practice as a Registered Nurse in the Commonwealth of Virginia, with American Heart Association (AHA) Health Care Provider BLS certification required.
Physical Demands

This role requires standing for prolonged periods, frequent bending, stooping, and traveling. The successful candidate will also possess proficient communicative, auditory, and visual skills, with the ability to write legibly and lift/push/pull objects over 100lbs.

Work Environment

The successful candidate will be exposed to a fast-paced healthcare environment, with potential exposure to fumes, chemicals, vibrations, humidity, cold, heat, noise, outdoor climate, blood/body fluids, and infectious disease.
