Job Opportunity

We are seeking a skilled and compassionate LPN Home Care Specialist to join our team at Specialty Home Care.

About the Role

  • Provide high-quality, patient-centered care to medically fragile patients in their homes.
  • Work collaboratively with our interdisciplinary team to develop and implement individualized care plans.
  • Conduct thorough assessments, develop and implement care plans, and provide ongoing care and support to patients.
  • Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination.
  • Participate in ongoing education and training to stay current with best practices and industry developments.

Requirements

  • 1-2 years of hospital experience, with a focus on acute care.
  • Hands-on experience with tracheotomy, in-home ventilator care, nebulizer management, G-tube management, and Foley care management.
  • Reliable transportation and good documentation skills.
  • Current, unencumbered license to practice as a LPN or RN in the District of Columbia.
  • Current CPR certification and a copy of updated PPD/Chest X-Ray.
