We are looking for a dedicated Targeted Case Manager to enhance our growing team. In this position, your primary duties will encompass:

Core Responsibilities:
  • Ensuring confidentiality and compliance with regulations
  • Documenting information for insurance requirements
  • Creating a supportive atmosphere for clients
  • Facilitating client recovery initiatives
  • Coordinating appointments and services for clients
  • Monitoring the well-being of clients
  • Connecting clients with essential resources and support networks
  • Assisting clients in achieving their goals
  • Conducting regular meetings with clients
  • Supporting care plans and providing educational assistance
Qualifications and Experience Required:
  • Bachelor's degree in social work or a related discipline
  • At least 1 year of post-graduate experience or a master's degree
  • Possession of a valid driver's license
  • Basic proficiency in computer skills
  • Excellent communication and documentation skills
  • Ability to handle crisis situations effectively
