Job Summary

Under the general direction of the Program Manager, but working collaboratively with the Senior Care Options (SCO) representative(s), the SCO Case Manager plays a vital role in the case management functioning of the agency.

A SCO Case Manager conducts assessments of the SCO client's needs and determines eligibility for any relevant services and resources.

The SCO Case Manager also develops service plans which would address the clients' needs and concerns based on the assessments.

This Position is best suited for one who can work autonomously, is empathetic and can multitask as the roles involves conducting home visits, family/provider meetings, telephonic contact and regular documentations.

Besides interacting with clients and their formal and informal supports, the SCO Case Manager works collaboratively within Boston Senior Home Care as well as with SCO representatives, vendors, potential referral sources and other community agencies.

Key Responsibilities
  • Conduct assessments of SCO clients' needs and determine eligibility for relevant services and resources.
  • Develop service plans to address clients' needs and concerns.
  • Work collaboratively with clients, their formal and informal supports, and other agencies.
  • Conduct home visits, family/provider meetings, and telephonic contact.
  • Regularly document client interactions and progress.
Requirements
  • Bachelor's Degree in social work, human services, nursing, psychology, sociology, or related field is preferred.
  • SW, LSW and relevant work experience preferred, but not required.
  • Bilingual in English/Cantonese/Mandarin.
