Position Overview

The Licensed Practical Nurse (LPN) in the role of Care Coordinator is a vital healthcare professional tasked with orchestrating the care delivered to qualified Medicare beneficiaries.

Key Responsibilities
  • Comprehensive Assessments: Conduct thorough evaluations of patients' health, encompassing physical, psychological, and social dimensions.
  • Medication Management: Perform medication reviews or reconciliations as needed or directed.
  • Health Monitoring: Observe patients' health status and responses to treatment plans, making adjustments as required.
  • Care Plan Development: Collaborate with patients, families, and healthcare professionals to create, implement, and regularly update personalized care plans.
  • Care Coordination: Facilitate communication among various healthcare providers and social services, ensuring seamless care transitions.
  • Patient Education: Inform beneficiaries and their support systems about chronic conditions, care interventions, and self-management techniques.
  • Documentation: Maintain precise and current medical records, detailing all patient interactions, care plans, and outcomes.
  • Follow-Up Care: Ensure patients receive necessary follow-up care with their primary care providers post-hospitalization or emergency visits.
  • Community Resource Identification: Recognize and refer beneficiaries to suitable community resources and support services.
  • Quality Improvement: Monitor and assess the effectiveness of care coordination activities, providing recommendations for enhancements.
Qualifications

The ideal candidate will possess:

  • Significant experience in care coordination for Medicare beneficiaries within a healthcare setting.
  • An Associate Degree and a Licensed Practical Nurse Certificate.
  • Strong critical thinking and communication skills.
  • Empathy for elderly patients with chronic conditions.
  • Ability to collaborate effectively with primary care teams and specialists.
  • Current CPR and first aid certification.
  • Arizona Licensed Nurse status.

This role requires ongoing professional development to remain informed about clinical treatments for patients with multiple chronic conditions and adherence to HIPAA regulations.
