Job Title: Community Justice Case Manager

Evergreen Treatment Services is seeking a highly motivated and compassionate Community Justice Case Manager to join our team. As a key member of our Community Justice Program, you will play a vital role in providing outreach and intensive case management services to individuals experiencing homelessness and behavioral health challenges.

Job Summary

This position is critical to the LEAD project, which aims to build community capacity to increase public health and safety while decreasing reliance on policing and the legal system. As a Community Justice Case Manager, you will provide direct services to an active case load of approximately 25-30 individuals, engaging participants on the street and at social service provider facilities to establish a working relationship and offer services.

Responsibilities
  • Provide outreach and intensive case management services to assigned clients, assessing their needs and developing individualized plans to address their challenges.
  • Engage participants in the criminal justice system, including court appearances and written communication, to advocate for their rights and interests.
  • Collaborate with LEAD partners, including law enforcement, social service providers, and community organizations, to ensure a comprehensive and coordinated response to the needs of participants.
  • Develop and maintain relationships with community resources, including housing providers, healthcare services, and employment opportunities, to support participants in achieving stability and improved quality of life.
  • Provide advocacy and support to participants, addressing issues related to housing, employment, healthcare, and other areas of need.
Requirements
  • An undergraduate degree in a field related to social services or behavioral health, or relevant lived experience.
  • Experience working with populations impacted by homelessness, behavioral health challenges, and trauma, from a harm reduction model of care.
  • Strong interpersonal and communication skills, with the ability to establish and maintain effective relationships with participants, partners, and community resources.
  • Ability to work independently and as part of a team, with a high level of initiative and a commitment to continuous learning and professional development.
