Job Title: Case Manager

Community Passageways is seeking a skilled Case Manager to join our team. As a Case Manager, you will play a critical role in providing direct services to individuals in need, including outreach, case management, and referrals.

Key Responsibilities:
  • Provide outreach and intensive case management services to assigned participants, including engaging participants, assessing their needs, and developing individualized service plans.
  • Collaborate with LEAD partners, community-based organizations, and other stakeholders to provide comprehensive support services.
  • Identify gaps and barriers in community resources and advocate for systemic changes to improve services.
  • Develop and maintain participant files, ensuring compliance with program, contract, and state requirements.
Requirements:
  • Case management experience and/or connection to community resources and systems.
  • Ability to engage respectfully and develop working relationships with participants, including those with lived experience of the criminal legal system.
  • Understanding of harm reduction and passion for serving individuals experiencing homelessness and co-occurring disorders.
  • Street outreach experience and skills in advocacy and support within the criminal legal system.
  • Computer literacy and ability to prioritize workload, complete tasks in a timely manner, and establish effective working relationships.
