Key Responsibilities:- Provide outreach and intensive case management services to assigned participants, including engaging participants, assessing their needs, and developing individualized service plans.
- Collaborate with LEAD partners, community-based organizations, and other stakeholders to provide comprehensive support services.
- Identify gaps and barriers in community resources and advocate for systemic changes to improve services.
- Develop and maintain participant files, ensuring compliance with program, contract, and state requirements.
- Case management experience and/or connection to community resources and systems.
- Ability to engage respectfully and develop working relationships with participants, including those with lived experience of the criminal legal system.
- Understanding of harm reduction and passion for serving individuals experiencing homelessness and co-occurring disorders.
- Street outreach experience and skills in advocacy and support within the criminal legal system.
- Computer literacy and ability to prioritize workload, complete tasks in a timely manner, and establish effective working relationships.
