Job Summary:

The Intake and Engagement Coordinator plays a vital role in facilitating the intake process, ensuring a smooth transition into services, and engaging participants in their care plans. This position requires close collaboration with referral sources and internal teams to link individuals to appropriate services, including case management, counseling, medical services, and other support.

Key Responsibilities:
  • Coordinate the intake process, including participant engagement and welcome, comprehensive intake assessments, service plan development, and consent and documentation.
  • Link participants to internal services, including appointment scheduling, referral and follow-up, service navigation, and barrier identification.
  • Provide ongoing engagement and support, including check-ins and monitoring, follow-up calls, outreach and engagement, and engagement reporting.
  • Assist in crisis management and problem resolution, including crisis response, conflict resolution, and problem-solving.
  • Collect and document participant interactions, referrals, and follow-up efforts, and track key performance indicators (KPIs) such as medication adherence, appointment attendance rates, engagement levels, and service completion rates.
Requirements:
  • An associate degree in social work, human services, psychology, or a related field is preferred.
  • A minimum of 1-2 years of experience in case management, social services, intake coordination, or a similar role is preferred.
  • Familiarity with intake procedures and participant engagement strategies is preferred.
  • Active, restriction-free Washington State Driver's License is required, with an acceptable driving record.
