Pharmacist Clinical Specialist

Dartmouth Health is seeking a skilled Clinical Pharmacist to join our team. This full-time position offers a competitive salary of $120,000 - $150,000 per year.
The successful candidate will engage with patients and healthcare professionals to optimize therapeutic outcomes. Key responsibilities include medication dispensing, monitoring patient drug therapies, and providing consultation and education.
Key Responsibilities:
- Gathers comprehensive medication histories and reconciles medication lists as applicable.
- Completes evidence-based medication reviews to ensure medications are necessary, safe, and cost-effective.
- Identifies and records suspected adverse drug reactions, adherence gaps, and barriers to patient adherence.
- Contacts providers when unsafe or inappropriate medication use is identified.
- Serves as a consultation resource for medical professionals and answers inquiries on drug therapy and products.
- Consults patients and caregivers on medication therapy when necessary.
- Assists in discharge planning and counsels patients and caregivers on proper drug administration.
- Contributes to the development and management of medication-related guidelines.
- Detects and reports medication errors and near misses.
- Ensures readiness for regulatory and accreditation surveys.
