Clinical Pharmacist Opportunity

We are seeking a highly motivated and experienced Clinical Pharmacist to join our team at D-H Lebanon-MHMH. As a Clinical Pharmacist, you will be responsible for providing medication therapy management services and improving patient outcomes in a fast-paced and dynamic environment.

About the Role

This is a full-time position that requires a minimum of 40 hours per week. The successful candidate will work closely with healthcare professionals to optimize medication therapy and prevent potential harm.

Essential Responsibilities
  • Gather comprehensive medication histories and reconcile medication lists as necessary.
  • Conduct thorough medication reviews to ensure medications are necessary, safe, and cost-effective.
  • Collaborate with healthcare professionals to optimize medication therapy and prevent potential harm.
  • Provide consultation and education to patients and other staff members on various aspects of medication therapy.
Qualifications and Requirements
  • Bachelor's degree in Pharmacy from an ACPE-accredited school.
  • Doctor of Pharmacy (PharmD) degree preferred.
  • Completion of a post-graduate residency and/or board certification is desirable.
  • Current licensure to practice in the state of NH is required, and willingness to obtain license reciprocity in multiple states is essential.
Salary and Benefits Package

The estimated annual salary for this position is $130,000 - $150,000, depending on experience. A competitive benefits package, including health insurance, retirement savings, and paid time off, is also offered.
