Clinical Care Specialist

Our Clinical Care Specialist role plays a pivotal part in delivering high-quality patient care. The successful candidate will join our team as a care team member, working collaboratively to provide professional care to patients with diverse conditions. This includes rooming patients, vital signs, updating history, medication list review, in-office procedures, telephone triage, wound care, and coordination of services with other team members.
Responsibilities and Qualifications
- Demonstrates competent clinical practice using clinical decision-making and critical thinking skills.
- Supports and promotes change to optimize patient outcomes and foster a productive professional practice environment.
- Engages patients and families in goal setting and planning, communicating the plan of care for input from both parties and colleagues for seamless care across shifts and settings.
- Utilizes current evidence and relevant national/specialty standards to guide practice and identifies areas for improvement.
- Salary range: $100,000 - $140,000 per year, based on experience.
