Job Summary: We are seeking a dedicated Nurse to join our team as a Health Advocate. The successful candidate will work closely with patients to develop personalized care plans, ensuring seamless transitions between healthcare providers and settings. This role requires strong communication and organizational skills, with a focus on improving health outcomes and reducing unnecessary healthcare utilization.

Responsibilities:
- Develop and implement individualized care plans for patients
- Coordinate care with healthcare providers and community services
- Educate patients on health conditions and self-care skills
- Facilitate patient empowerment and quality of life
- Collaborate with healthcare teams to ensure seamless care transitions

Requirements:
- Bachelor of Science degree in Nursing required
- Current, unencumbered DC Registered Nurse license with CPR certification
- Knowledge of primary care and health maintenance
- Experience educating patients with chronic health conditions preferred
- Strong verbal and written communication skills
- Ability to work with computers and electronic health records
- Strong organizational skills with multitasking ability
