Position Purpose: Responsible for assessing, planning, implementing, and evaluating the nursing care provided to residents in a skilled nursing environment. Ensures continuity of care by delivering direct care and supervising the nursing staff, including CNAs and LPNs. Engages in resident and family education while advocating for residents' needs in alignment with the facility's mission.

Educational Requirements: Graduation from an accredited nursing program.

Experience Required: A minimum of one year of supervisory experience in long-term care as an RN, with strong leadership and clinical skills.

Professional Licensure and Certification Required: Current, unencumbered RN license in NY state. Valid CPR and IV certifications.

Interpersonal Relationships: Effectively communicates with residents, families, visitors, colleagues, physicians, and other staff members.

Essential Functions:

  • Oversees the nursing department's clinical activities during assigned shifts, ensuring compliance with current regulations governing long-term care facilities.
  • Ensures thorough completion of new admissions and pertinent assessments.
  • Addresses emergent resident needs and anticipates changes in conditions, making appropriate clinical decisions.
  • Acts as a resource for staff in clinical decision-making and delegates responsibilities as necessary.
  • Participates in inspections conducted by authorized agencies.
  • Assures appropriate assessment of residents and validates nursing staff's clinical decisions.
  • Contributes to the development, implementation, and communication of residents' care plans.
  • Conducts regular rounds on nursing units to ensure compliance with established nursing standards and that residents' needs are met.
  • Monitors nursing staff documentation procedures and addresses any issues with employees, reporting to the Director of Nursing.
  • Performs any additional duties as assigned by the Director of Nursing.

Personnel Functions:

  1. Adheres to work assignments and schedules to complete tasks efficiently.
  2. Completes all assigned tasks in accordance with established policies and procedures.
  3. Collaborates with interdepartmental personnel to ensure services meet residents' needs.
  4. Follows attendance rules as outlined during orientation.
  5. Recognizes staffing needs and facilitates appropriate staff assignments.
  6. Meets regularly with staff to identify and address problem areas and improve services.
  7. Functions as a staff nurse or provides direct care as needed.
  8. Ensures fair administration of disciplinary actions without bias.

Duties & Responsibilities:

  • Punctually reports to work, ready to start the shift.
  • Maintains a professional appearance in compliance with the facility's dress code.
  • Provides proper notification in case of lateness or absence.
