About the Role

We are seeking a highly skilled and compassionate Patient Navigator to join our team at Hackensack Meridian Health. As a Patient Navigator, you will play a critical role in ensuring that our patients receive the highest level of care and support throughout their journey with us.

Key Responsibilities
  • Advocate and Educator: Serve as a patient advocate and educator, providing guidance and support to patients and their families throughout the surgery and cancer continuum.
  • Communication Facilitator: Facilitate communication between patients and healthcare providers, ensuring that patients receive timely and accurate information about their care.
  • Resource Coordinator: Connect patients with resources and social support systems, helping them to navigate the healthcare system and access the services they need.
  • Clinical Expert: Provide competent clinical care to patients during office visits, and serve as a content expert in the specialty practice in which they are responsible.
  • Problem Solver: Identify and address barriers to care that might prevent patients from receiving timely and appropriate treatment.
Requirements
  • Education: RN (Associates, Diploma, BSN)
  • Experience: Minimum of 3 years of nursing experience in a related field (Surgery/Oncology)
  • Skills: Good computer skills, excellent written and verbal communication skills, strong organizational skills, and strong critical thinking skills
  • Licenses and Certifications: NJ State Professional Registered Nurse License
