Patient Care Coordinator

We are seeking a highly skilled and compassionate Patient Services Coordinator to join our team at EVO Care Group. As a key member of our care coordination team, you will play a vital role in ensuring the seamless delivery of high-quality patient care.
Key Responsibilities:- Assign and schedule caregivers for clients in their home or in facilities, confirming availability and acceptance of assignments.
- Initiate the intake process, developing patient profiles to identify and facilitate the best patient-caregiver match.
- Maintain accurate scheduling records, including visit confirmation using HHA exchange and/or Salesforce.
- Communicate effectively with caregivers, clients, and families regarding patient information and caregiver assignments.
- Respond to voicemail and email messages in a timely and professional manner.
- Document all calls and follow proper telephone follow-up procedures.
- Collaborate with the clinical team to convey reported changes in condition or inquiries requiring clinical direction.
- Work closely with the HR team to assist in the recruitment of ancillary/paraprofessional personnel.
- Ensure compliance with all Federal, State, and City regulations, as well as agency Policies and Procedures.
- Adhere to the terms and conditions of all contractual relationships.
- Communicate with Contract case managers/schedulers regarding changes, missed visits, and client concerns.
- HOME CARE experience (preferred)
- Spanish speaking skills (preferred)
- Ability to work independently and within a team
- Strong interpersonal communication skills
- Excellent customer service skills, with empathy and patience when working with the elderly population
