Job Title:

Senior Housing Outreach Specialist - Harm Reduction

Job Summary:

Goddard Riverside is seeking a highly skilled Senior Housing Outreach Specialist to join our team. The successful candidate will work creatively and effectively in the community to engage individuals living on the street through outreach by utilizing a harm reduction approach. This role will also serve as a case manager for adults experiencing street homelessness, with the goal of placing them into transitional and permanent housing.

Key Responsibilities:
  • Perform targeted joint outreach and engagement in areas known to have high substance use within the unsheltered population
  • Provide harm reduction supplies, education, and resources to unsheltered clients
  • Assist with team trainings in harm reduction practices and serve as the Harm Reduction Champion for the team
  • Work as part of an inter-disciplinary team to promote wellness
  • Maintain an active resource directory of harm reduction resources in the catchment area and act as liaison with these resources and the team
  • Serve as case manager for homeless adults who use substances, with a goal of placing them in transitional and permanent housing
  • Perform intake, assessment, service planning, advocacy, referral, and escort services
  • Maintain client case records and enter data in computer database using handheld and desktop computers
  • Provide follow-up case management for clients placed in housing, to promote retention and document outcomes
  • Attend staff meetings, case conferences, trainings, and supervision meetings; participate in program evaluation and improvement
  • Communicate with evening/weekend teams and with staff of other agencies to coordinate client services
  • Participate in outreach trips to identify and engage with homeless clients
  • Drive outreach vehicles and use public transportation as necessary
  • Participate in the annual HOPE count and quarterly counts of homeless individuals living on the street, 4 overnight counts completed annually
  • Assist with onboarding of new Housing Outreach Specialists
Requirements:
  • Bachelor's Degree preferred
  • Prior experience with the homeless, substance use, or mental health preferred
  • Valid driver's license and a driving record that meets the agency's auto insurance guidelines (including at least one year as a licensed driver and no suspensions within the last 3 years)
  • Flexibility in work hours; available to work evening, overnight, and weekend shifts when required
Skills and Abilities:
  • Experience in Harm Reduction preferred
  • Ability to be flexible in a fast-paced work environment
  • Strong written and verbal skills
  • Team player
  • Good judgment
Computer Skills:
  • Able to work in the Microsoft Office Suite, especially proficient in Word and Outlook
  • Able to use or learn other electronic databases, programs, and funder-related software, such as Mocapp, Streetsmart, HMIS, and AWARDS
Work Environment:

Indoors and outdoors; all weather
