Job Title: Senior Housing Outreach Specialist

Goddard Riverside is seeking a highly motivated and compassionate Senior Housing Outreach Specialist to join our team. As a key member of our outreach team, you will work closely with individuals experiencing homelessness, providing case management services and connecting them with permanent housing solutions.

Key Responsibilities:

  • Provide case management services to homeless adults, including intake, assessment, service planning, and advocacy
  • Collaborate with community partners and healthcare agencies to achieve housing placement goals
  • Conduct outreach trips to identify and engage with homeless clients
  • Maintain accurate client records and enter data into our computer database
  • Participate in program evaluation and improvement efforts

Requirements:

  • Bachelor's degree preferred
  • Prior experience working with homeless and/or mentally ill populations
  • Valid driver's license and a driving record that meets our agency's auto insurance guidelines
  • Flexibility in work hours, including evening, overnight, and weekend shifts
