Role Overview

The Health Navigation Specialist plays a crucial role in assisting members to implement sustainable lifestyle modifications aimed at enhancing their overall health. Collaborating with Care Management Registered Nurses, this position focuses on developing tailored wellness strategies, providing educational reinforcement, and delivering continuous support to help members achieve their health objectives.

Key Responsibilities:

  • Work collaboratively with team members to ensure members have access to vital services.
  • Engage with patients and their families to promote involvement and address challenges throughout their healthcare experience.
  • Encourage members to embrace positive changes in nutrition, physical activity, stress management, and safety protocols.
  • Manage care referrals for both registered and non-registered participants.
  • Evaluate members' needs during telephonic and/or digital interactions and refer them for specialized medical assistance as needed.
  • Utilize motivational interviewing techniques to support members in setting and achieving mutually agreed-upon health goals.
  • Proactively reach out to eligible members to promote program enrollment.
  • Distribute approved health education resources to participants through various channels.
  • Encourage member participation in preventive care, health screenings, and wellness initiatives offered by the organization or its partners.
  • Connect patients and their families with community resources and follow up to ensure their needs are met.
  • Perform tasks under direct supervision, addressing basic issues and escalating more complex matters as necessary.
  • Document communications, actions, and other relevant data in appropriate systems for record-keeping and auditing purposes.

Qualifications:

  • An Associate's degree in a healthcare-related discipline.
  • A minimum of 2 years of experience in healthcare settings (insurance, customer service, direct patient care) or related fields is essential for success in this role.
  • Strong interpersonal communication skills are vital as this role involves direct interaction with members.
  • Ability to adapt to various situations and work collaboratively with both members and the Care Coordination team.
  • Effective verbal and written communication skills are necessary for engaging with members and the team.

Equivalent combinations of education, experience, or training deemed acceptable by Human Resources may be considered.
