Job Summary

We are seeking a highly skilled Care Coordinator to join our team at Caring People Alliance. The successful candidate will work collaboratively with our case management team to provide exceptional care coordination services to children and youth with special health care needs and their families.

Key Responsibilities
  • Work with patients and families to develop and implement personalized care plans that address their unique needs and goals.
  • Assess patients' unmet health and social needs, and develop strategies to address these needs in collaboration with the patient, family, and healthcare providers.
  • Monitor patients' progress, evaluate the effectiveness of care plans, and make adjustments as needed.
  • Collaborate with healthcare providers, community resources, and other stakeholders to ensure seamless care coordination and optimal health outcomes.
  • Provide education and support to patients and families to empower them to manage their health and navigate the healthcare system.
  • Identify and address barriers to care, including financial, practical, and social challenges.
  • Develop and maintain relationships with community resources and healthcare providers to ensure access to necessary services and support.
  • Participate in regular team meetings and contribute to the development of care coordination strategies and best practices.
Requirements
  • Bachelor's degree in healthcare or a related field.
  • Three years of experience in clinical or community resource settings, care coordination, and/or case management.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations.
  • Strong problem-solving and critical thinking skills, with the ability to analyze complex situations and develop effective solutions.
  • Proficiency in Microsoft Office and other relevant software applications.
  • Valid driver's license and clean driving record.
