Position Overview

Role Summary:

The Care Coordinator plays a crucial role in supporting individuals within COMHAR's Outpatient Programs by managing and facilitating care. This position connects new clients to outpatient services through the intake process, provides essential information and education, links individuals to necessary resources, fosters communication with partner and provider agencies, and motivates clients to reach their recovery objectives.

Key Responsibilities:

  • Deliver information, education, and encouragement while coordinating care throughout various recovery phases, including pre-recovery engagement, recovery initiation, stabilization, and ongoing maintenance.
  • Enhance communication with primary care and other medical providers to support integrated healthcare.
  • Maintain partnerships with community organizations, acting as a liaison to coordinate care by facilitating communication, obtaining necessary consents, addressing service barriers, and participating in linkage, treatment team, and care coordination meetings as required.
  • Conduct triage and manage the enrollment process for individuals seeking services at the Welcome Center and/or Open Access points.
  • Engage in outreach initiatives, including mobile outreach, to connect individuals with suitable services.
  • Proactively identify and provide linkages to community-based resources that support the individual's recovery journey, such as medical providers, recovery communities, educational, vocational, social, cultural, spiritual resources, mutual self-help groups, and professional services.
  • Collaborate with individuals and treatment teams (therapists, psychiatric practitioners, nurse supervisors, peer specialists, and case managers) regarding recovery planning needs related to resources or care coordination.
  • Assist individuals in crisis within behavioral health care settings, which may involve accompanying them to Crisis Response Centers or delivering necessary documentation to facilitate crisis services.
  • Ensure timely completion of required documentation, including submission of care coordination encounters with individuals, families, or service providers in the electronic health record.
  • Support therapists with outreach and caseload management when care coordination is necessary.
  • Adhere to a flexible schedule that accommodates evening and weekend hours as needed to address individual requirements.
  • Provide support to multiple outpatient programs as needed, including but not limited to Enhanced Outpatient, Specialized Services, and Latino Treatment Program.
  • Perform additional duties as assigned by the supervisor or program leadership to enhance care coordination and integrated care.

Qualifications:

  • Bachelor's degree in social work, counseling, psychology, sociology, gerontology, counseling education, or a related field.
  • A minimum of 1 year of experience in the health or human service field, with a preference for experience in behavioral health or substance use.

Skills Required:

  • Basic understanding of clerical procedures and systems, such as recordkeeping and filing.
  • Ability to work independently and collaboratively within teams.
  • Capacity to apply common sense and understanding to execute instructions in written, oral, or diagram form.
  • Aptitude for addressing problems involving several concrete variables in standardized situations.
  • Understanding and respect for each individual's unique recovery path, providing person-centered, trauma-informed, and strength-based support.
  • Strong organizational skills.
  • Working knowledge of the recovery/treatment system.
  • Flexibility and adaptability in managing changing priorities in a fast-paced environment.
  • Professionalism and ethical service at all times.
  • Computer literacy is essential, including effective use of electronic health records, video conferencing, email, Microsoft Office products, and other necessary technologies.
