Position Overview:
The Care Coordinator plays a pivotal role in facilitating health and human services at COMHAR, aimed at empowering individuals, families, and communities.

Key Responsibilities:
  • Assist individuals in navigating their recovery journey by providing comprehensive care coordination throughout various stages, including pre-recovery engagement, recovery initiation, stabilization, and maintenance.
  • Foster effective communication with primary care and medical providers to ensure integrated healthcare delivery.
  • Maintain collaborative relationships with community partners, serving as a liaison to enhance care coordination through effective communication, consent management, and participation in treatment team meetings.
  • Conduct initial assessments and facilitate the enrollment process for individuals seeking services at designated access points.
  • Engage in proactive outreach efforts, including mobile outreach, to connect individuals with essential services.
  • Identify and link individuals to community resources that support their recovery, including medical, educational, vocational, and social services.
  • Collaborate with individuals and treatment teams to address recovery planning needs and resource coordination.
  • Provide support to individuals in crisis situations within behavioral health settings, which may involve accompanying them to crisis response facilities.
  • Ensure timely and accurate documentation of care coordination encounters in the electronic health record system.
  • Assist therapists with outreach and case management as needed.
  • Maintain a flexible schedule to accommodate evening and weekend hours as required by individual needs.
  • Support multiple outpatient programs, including Enhanced Outpatient and Specialized Services, as necessary.
  • Perform additional duties as assigned by supervisory or program leadership to enhance care coordination and integrated service delivery.

Qualifications:
  • Bachelor's degree in social work, counseling, psychology, sociology, gerontology, or a related field.
  • A minimum of 1 year of experience in the health or human services sector, preferably in behavioral health or substance use.
  • Valid clearances as required by state regulations.
  • Proficiency in using electronic health records and other relevant technologies.

  • Strong organizational and communication skills.
  • Ability to work independently and collaboratively within a team environment.
  • Flexibility to adapt to changing priorities in a dynamic setting.

Work Environment:
The Care Coordinator will primarily operate within a fast-paced outpatient clinic environment, with occasional meetings in community settings.
