We are seeking a highly skilled and compassionate Patient Navigator to join our team at Thomas Jefferson University & Jefferson Health. Key Responsibilities- Scheduling and Coordination: Schedule peer-to-peer and obtain prior authorizations for fellows and sickle cell clinic patients.
- Triage and Patient Care: Triage new and established patients' medical issues, provide patient care that reflects a family-centered approach, and assist with patient education.
- Communication and Collaboration: Develop and maintain effective physician-patient communication, facilitate timely communication between departmental and referring physicians, and provide clinical and administrative support to the Hematology team.
- Administrative Support: Provide backup support for patient service representatives, assist patients with pre- and post-procedure questions, and interact with coworkers, visitors, and other staff consistent with the core values of the University.
- Care Coordination: Coordinate patient care for new patients by working closely with referring physician offices to provide complete medical records and radiologic images for physician review.
- Record Management: Determine, obtain, and organize pertinent records for new patient office visits, follow-up office visits, and procedures.
- Follow-up Care: Coordinate all follow-up care for established patients in accordance with practice guidelines and office policy.
- Precertification: Assist in the precertification process for all patient services.
- Chart Accuracy: Assure accuracy and integrity of patient charts.
- Referring Physician Communication: Triage referring physician telephone calls and assure timely and efficient response.
- Appointment Coordination: Coordinate patient appointments for office visits and procedures as directed by physicians.
- Patient Education: Provide courteous and thorough patient education.
We require a high school diploma or equivalent and experience in a hospital-based physician practice.
