General Summary:
We are seeking dedicated Community-Based Case Managers to serve in the Southern Maine area.
The Case Manager plays a vital role in delivering comprehensive case management services across various programs, including Community Integration, Behavioral Health Home, Opiate Health Home, and Targeted Case Management. Responsibilities encompass collaborating with a multidisciplinary team to formulate, execute, and assess service plans; establishing both short- and long-term objectives; identifying resources and facilitating placements; advocating for clients' needs; and maintaining detailed case plans and documentation. Once a caseload is established, the Case Manager is expected to meet billing requirements. The role also involves acting as a consultant, educator, and advocate, with performance evaluated through reports, observations, and supervisory assessments.
Key Responsibilities:
- Craft an Individual Service Plan (ISP) for each enrolled youth, encompassing intake, assessment, treatment, and ongoing consultations, addressing needs across seven domains.
- Monitor and adjust individual service plans for clients and their families as necessary.
- Engage in discussions regarding client progress with all stakeholders involved in the service plan, guiding clients on their responsibilities and advancements towards their ISP.
- Assist clients and their families in identifying suitable service providers, advocating for their needs, and coordinating service delivery with relevant agencies.
- Conduct regular meetings with clients to ensure ongoing support.
- Review and manage case records, preparing timely reports and summaries on client progress. Document and report any unusual situations that may impact client participation and placement.
- Ensure all clinical records comply with agency, program, licensing, and clinical standards.
- Provide necessary statistics and client data to meet contractual and billing obligations.
- Engage in clinical supervision, training sessions, and team meetings.
- Represent the organization professionally within the community.
- Perform additional duties as assigned.
Confidentiality:
Consistently adhere to all policies and procedures regarding the confidentiality of client, employee, and agency information.
Supervisory Responsibilities:
This position does not include supervisory responsibilities.
Work Environment:
Office-based tasks can be conducted from various locations within the service area. Local travel is required to meet client needs.
Physical Requirements:
Physical demands are consistent with routine office work, including extensive computer use and local travel. Reasonable accommodations may be made for individuals with disabilities to perform essential functions.
Qualifications and Skills:
- MHRT-C Certification is required.
- LSW or LSX is preferred.
- Experience working with individuals of all ages facing mental health and/or substance use challenges.
- Ability to collaborate effectively as part of a team.
- Strong verbal, written, and interpersonal communication skills.
- Reliable transportation and capability to travel locally.
- Flexibility to work varied hours to accommodate client needs.
- Ability to manage stressful situations and interact positively with clients and colleagues.
