Overview:

The Patient Care Coordination Specialist plays a pivotal role in overseeing all direct nursing services provided to patients and residents, working closely with the nursing staff on the unit.

This position serves as a specialist in geriatric nursing, acting as a valuable resource for unit staff and other departments within the institution as required.

Job Details:

MINIMUM QUALIFICATIONS:

Education:

Graduate of an accredited Nursing Program or an accredited College with a Registered Nurse degree.

Experience/Skills:

A minimum of three (3) years of experience in long-term care or a combination of long-term and acute care settings.

Must possess a thorough understanding of nursing and medical practices, procedures, and the laws, regulations, and guidelines applicable to long-term care.

Licensure:

Must hold a valid Registered Nurse license in New York State.

Posted Salary Range:

USD $67.00 /Hr.

The compensation for this role ranges from $60.00 to $67.00 per hour. This range is provided as a good faith estimate, and actual pay will depend on various factors, including the candidate's qualifications, skills, competencies, and experience.

Please note that the salary range does not include bonuses, incentive pay, differential pay, or other forms of compensation that may apply to this position, nor does it account for the value of benefits.
