Job Summary

Key Responsibilities
  • Coordinate patient care and scheduling to ensure seamless delivery of services
  • Communicate effectively with patients, families, and healthcare teams to address health concerns and provide support
  • Develop and implement personalized care plans to meet the unique needs of each patient
  • Collaborate with clinical teams to ensure timely and effective care coordination
  • Provide education and resources to patients and families to promote healthy behaviors and disease management
Requirements
  • High school diploma or equivalent required; associate's degree preferred
  • Minimum 3-5 years of healthcare experience, including customer service, patient relations, and/or hospitality concierge experience
  • Strong communication and interpersonal skills, with ability to work effectively with diverse populations
  • Ability to work in a fast-paced environment and prioritize multiple tasks and responsibilities
  • Proficiency in Microsoft Office applications, including Outlook, Word, and Excel
