Care Management Expertise

Elevance Health is seeking a skilled Nurse Care Coordinator to join our team. As a key member of our care management team, you will be responsible for assessing, developing, implementing, and evaluating care plans for members with complex and chronic care needs.

Key Responsibilities:

  • Conduct thorough assessments to identify individual needs and develop personalized care plans.
  • Collaborate with healthcare providers, members, and family members to ensure seamless care coordination.
  • Monitor and evaluate the effectiveness of care plans, making adjustments as needed.
  • Interface with Medical Directors and Physician Advisors to develop comprehensive care management treatment plans.

Requirements:

  • BA/BS in a health-related field and a minimum of 5 years of clinical experience.
  • Current, unrestricted RN license in applicable state(s) required.
