About the Role

This is a unique opportunity to join Community Health Plan of Washington as a Complex Case Manager. As a key member of our care management team, you will play a vital role in coordinating services for our members with complex medical and behavioral conditions. This position requires a high level of organization, communication, and interpersonal skills, as well as the ability to work independently and collaboratively with various stakeholders.

As a Complex Case Manager, you will be responsible for assessing the needs of our members, developing and implementing individualized care plans, and collaborating with healthcare providers to ensure timely and effective care delivery. Your strong analytical skills and ability to interpret data will enable you to identify areas for improvement and develop solutions to drive better health outcomes.

The ideal candidate will possess a Bachelor's degree in nursing or a related field, a current license in the state of Washington, and at least one year of case management experience. A case management certification is preferred but not required. The salary range for this position is approximately $ $83,500 - $120,000 per annum, depending on your qualifications and experience.
