Job Description

We are seeking a highly skilled Senior Case Manager RN to join our team at Professional Case Management. As a Senior Case Manager RN, you will be responsible for overseeing the provision of care services to clients in their homes. This role requires strong leadership and clinical skills, as well as the ability to maintain accurate records and collaborate with interdisciplinary teams.

Key Responsibilities
  • Develop and implement individualized care plans that meet the unique needs of each client
  • Supervise and evaluate the performance of RN Case Managers, providing constructive feedback and coaching as needed
  • Maintain up-to-date knowledge of agency policies, state regulations, and industry best practices
  • Collaborate with staffing coordinators to optimize personnel scheduling and resource allocation
  • Participate in performance improvement initiatives, including quarterly medical record reviews and data collection
Requirements and Qualifications
  • Active RN license in good standing
  • Minimum 2 years of nursing experience, including 1 year in home care or a closely related field
  • 1 year of supervisory and/or case management experience preferred
  • Demonstrated ability to work independently and as part of a team
