Job Description

Tennessee Family Solutions is seeking a skilled Certified Nursing Assistant to join our team.

Key Responsibilities:
  • Follow established work assignments and schedules to complete tasks efficiently.
  • Perform all assigned tasks in accordance with established policies and procedures, as instructed by the supervisor.
  • Notify the facility of any late or absent work according to specified call-in procedures.
  • Collaborate with other facility personnel to ensure that nursing services meet the needs of our residents.
  • Contribute to a warm, personal, and positive environment throughout the unit and shift.
  • Participate in and receive the nursing report upon reporting for duty.
  • Assist residents with daily dental and mouth care.
  • Provide back rubs as instructed.
  • Assist residents with dressing/undressing as necessary.
  • Assist residents with hair care functions (combing, brushing, shampooing, etc.).
  • Assist residents with nail care (clipping, trimming, and cleaning the finger/toenails).
  • Shave residents as needed or requested.
  • Keep residents dry (change gown, clothing, linen, etc., when it becomes wet or soiled).
  • Make beds.
  • Position bedfast residents in the correct and comfortable position.
  • Change bed linens. Keep linens tight to avoid wrinkles from forming under the resident.
  • Put extra covers on beds as requested.
  • Assist a resident with bowel and bladder functions.
  • Maintain intake and output records as instructed.
  • Check and report bowel movements and character of stools as instructed.
  • Assist with bowel and bladder training program.
  • Assist residents in preparing for an activity and social programs.
  • Assist in transporting residents to/from appointments, activity, and social programs as necessary.
  • Assist with lifting, turning, moving, positioning, and transporting residents into and out of beds, chairs, bathtubs, wheelchairs, lifts, etc.
  • Answer resident calls promptly.
  • Assist residents to walk with or without self-help devices as instructed.
  • Perform restorative and rehabilitative procedures as instructed.
  • Provide eye and ear care.
  • Measure and record temperatures, pulse, and respiration as instructed.
  • Weight and measure residents as instructed.
  • Assist with the application of nonsterile warm/cold compresses.
  • Assist with the care of a dying resident.
  • Provide post-mortem care as instructed.
  • Check residents routinely to assure that their personal care needs are being met.
  • Observe and report the presence of pressure areas and skin breakdowns to prevent bedsores.
  • Provide daily personal care.
  • Give alcohol sponge baths.
  • Provide daily Range of Motion Exercises as requested.
  • Turn bedfast residents at least every two hours.
  • Observe disoriented and comatose residents. Record and report data as instructed.
  • Watch for and report any change in room temperature, ventilation, lighting, etc.
  • Prepare residents for meals (take to the bathroom, wash hands, comb hair, raise bed, take to/from dining room, etc.).
  • Serve food trays. Assist with feeding as indicated.
  • Assist residents with identifying food arrangements (informing residents with sight problems, what foods are on the tray, where it is located, if it is hot/cold, etc.).
  • Record the residents' food/fluid intake. Report changes in the resident's eating habits.
  • Keep resident water pitchers clean and filled with fresh water and within easy reach of the resident.
  • Serve between meal and bedtime snacks as directed.
  • Perform after meal care (remove trays, clean resident's hands, face, clothing, take to the bathroom, brush teeth, clean dentures, etc.).
  • Attend and participate in scheduled training and educational classes.
  • Keep floors dry. Report spills immediately.
  • Keep excess supplies and equipment off the floor. Store in designated areas.
  • Keep a resident's personal possession off the floor and properly stored.
  • Follow established smoking regulations. Report all violations.
  • Report all hazardous conditions and equipment to the Nurse House Manager or Supervisor immediately.
  • Follow established safety precautions in the performance of all duties.
  • Report all safety violations.
  • Wash hands before and after performing any service for the resident.
  • Wash wheelchairs, walkers, etc., as instructed.
  • Clean, disinfect, and return all personal care equipment to its designated storage area after each use.
  • Perform routine housekeeping duties (clean bedrails, nightstand, etc., that relate to nursing care procedures).
  • Wash hands before entering and leaving an isolation room/area.
  • Report any communicable or infectious disease that you contact with the Director of Nursing or designee.
  • Follow established isolation precautions and procedures.
  • Use gowns and gloves as indicated.
  • Operate all equipment in a safe manner.
  • Report defective equipment to the Nurse House Manager or Supervisor.
  • Inform the Nurse House Manager or Supervisor of your equipment and supply needs.
  • Review care plans daily to ensure that you are providing appropriate nursing care.
  • Inform the Nurse House Manager or Supervisor of any changes that need to be made on the care plan.
  • Ensure that your notes reflect that the care plan is being followed.
  • Maintain the confidentiality of all personal care information.
  • Ensure that you treat all residents fairly and with kindness, dignity, and respect.
  • Ensure that all nursing care is provided in privacy.
  • Knock before entering the resident's room.
  • Report all grievances and complaints made by the resident to the Nurse House Manager or Supervisor.
  • Report all accidents and incidents you observe on the shift that they occur.
  • Report all changes in the resident's condition as soon as practical.
  • When processing a new admission, ensure that the resident's room is ready, greet and escort residents to their room, and introduce residents to other residents and personnel as appropriate.
  • Inventory and mark the resident's personal possessions.
  • Assist residents with packing their personal possessions when they are being transferred to a new room or when being discharged.
  • Assist residents with loading/unloading/packing/unpacking as necessary.
  • Notify the Nurse House Manager/ Supervisor, immediately of any resident leaving/missing from the home.
