Home-Based Clinical Specialist

We are looking for a skilled healthcare professional to join our HouseCalls team in Yakima, WA.
As a member of our team, you will conduct in-home assessments and provide preventive care to patients, helping to identify and reduce health risks and improve overall well-being.
About the Role:
You will have the opportunity to work with a high-end medical service provider, delivering compassionate care and leading-edge solutions to vulnerable patient populations.
Key Responsibilities:
- Conduct in-depth home visits, assessing patients' medical history, medications, vital signs, and overall health.
- Perform screenings and tests to identify potential health issues and develop effective treatment plans.
- Communicate findings with patients' primary care physicians to ensure seamless care coordination.
- Respond to urgent and emergent situations, providing immediate support and intervention when needed.
- Educate patients on disease processes, medication adherence, and healthy lifestyle choices.
Requirements:
- Current NP or PA licensure or ability to obtain prior to start date.
- Active ANCC or AANP national certification in relevant specialty areas, or ability to obtain certification by start date.
- Ability to meet physical demands of the role, including lifting heavy bags, navigating various environments, and performing comprehensive assessments.
- Reliable transportation for home visit assessments.
Estimated Salary Range:$88,000 - $173,200 annually, based on education, experience, certifications, and local market conditions.
